Productivity, efficiency, and effectiveness

Colleagues,

I have enjoyed reading the discussion about measuring what we do and how efficiently we operate.  What I would like to add to this discussion is two aspects:
1)  Are we measuring whether the therapy was necessary (i.e. appropriateness criteria and use of protocols as a tool to ensure appropriateness), and
2)  Are we measuring the outcome of our therapy. 

We all know that we can be efficient in the delivery of a therapy, but we also need to be mindful that efficiency doesn't equate to effectiveness.

I'd like to continue this discussion by asking everyone to reply as to how effective your protocols have been to ensure the appropriate delivery of therapeutics and what outcome metrics you are utilizing to measure what impact you're having.
