We had two seperate occurrences last summer whereby our Oxy-Totes (aka Grab & Go) self contained E-O2 cylinder/regulator systems showed the tank was full @ 2000psig and the flow was appropriate but both patients SpO2 rapidly declined and the patient decompensated. After switching to wall O2, the SpO2 rapidly climbed and the patients improved clinically. So------a long investigation resulted in the fact that mud daubers were building nests in the O2 outlet of the self contained E-tank system somewhere during the post transfilling process. We now have protective caps (still with a safety vent hole) placed on the tanks post transfilling from our oxygen supplier. Now summer is back and for states that have mud daubers, beware as the female mud dauber can shoot mud in the tiniest of aperatures. Just when you thought you had seen it all------
