Yes.  My hospital monitors it as a patient safety event.  A report gets filed with every one that happens either by the Nurse or RT that witnessed it.

​Yes we do. We track accidental or self extubations and outcomes.

Hello.  I work in a Children's Hospital and we monitor unplanned extubations in our Pediatric ICU and our Neonatal ICU.
