Vapotherm

Hi everyone.

I had an inservice recently regarding the Vapotherm High Flow Oxygen device. (Hi-VNI) I was just curious if anyone uses it and how you like it? Have you had a positive patient response to the Vapotherm? And if you have used both which high flow system do you like the best. I am just trying to get some feedback. We currently use the high flow flowmeter system (up to 50 liters) and the F & P humidifier with a blender. I appreciate the feedback!

I have a lot of experience with Vapotherm System and the F & P units as well using OPTi flow

 The primary clinical difference is that with Vapotherm you actually are able to provide proper humidification at 37° 

 The second major difference is that you do tend to wash out that space with the smaller cannula size

With other systems making the attempt to provide the proper temperature in order to achieve  the  therapeutic range of humidification is difficult when you consider condensation and rain out in a heated wire circuit. As this routinely occurs, RT's tend to turn down the temperature and thus provide sub -optimal humidification

 From our training, we know that sub optimal humidification when delivering a cold gas source has an immediate negative effect and thus can complicate the care  and prolong the stay and treatment process 

 Lastly safety should be considered when using other systems outside of Vapotherm....their location of pop off valve's and how much CPAP pressure are their systems actually creating that we are not measuring due to their large bore cannula style ?

So the total cost of using Vapotherm versus the actual price should be considered, the price may be more, but your cost more than likely will be less.

vapotherm is a good system. it does provide slightly different therapy compared to optiflow system in regard to:

water saturation is higher with vapotherm, more humidity, using the water jacket concept

interfaces provides jet-like flows (small-bore) that (according to the manufacturer) supposed to offer better and faster dead space washout, in contrast to optiflow that has the large-bore interface

vapotherm is relatively more expensive tx than optiflow

Hey I've had quite a bit of experience with Vapotherm's system.  They are nice, but break down constantly.  Plus the parts are rather expensive to order (the Vapotherm circuit I mean).  Opti-flow does the same thing, cheaper, and you won't have to send it off to be fixed all the time.
